By registering your child for Vacation Bible School, you acknowledge and agree to the following:

I give permission for my child to participate in all VBS activities, including indoor and outdoor programming, games, and group events. I understand that while all reasonable precautions will be taken to ensure safety, participation in activities may involve inherent risks.

I certify that my child is in good health and able to participate. I have disclosed any relevant medical conditions, allergies, or special needs during registration. I understand that it is my responsibility to provide any necessary medications, clearly labeled with instructions. I acknowledge that VBS staff and volunteers are not responsible for administering medication unless otherwise arranged in advance.

In the event of a minor injury, I authorize New Life Fellowship staff to provide basic first aid. In the case of a more serious injury or medical emergency, I give permission for emergency medical treatment to be administered, including transportation to a medical facility if necessary. I understand that every effort will be made to contact me or my emergency contact.

I release and hold harmless New Life Fellowship, its staff, and volunteers from liability for any injury or illness that may occur as a result of participation, except in cases of gross negligence.

I also give permission for my child to be photographed or recorded during VBS activities for ministry and promotional use, unless otherwise noted in writing.
